Sample Taker Database

In order for Sample Takers / Practice Managers to get their results they need to submit a completed registration form to QA ( submission details can be found on the form). This registration process is a one-off activity.

Download instructions for databases (overview)

1. Connect to the Internet

2. Open the NWCSQARC website (www.nwcsqarc.org.uk)

3. Navigate to Sample Taker page (link halfway down left side of Home page)

4. Right-click on the appropriate link ( this is detailed in the confirmation letter ). For example, Sample Taker Database 010107 to 311207 v070408 - this link will download the Sample Taker Database covering the period 01 Jan 2007 to 31 Dec 2007 which contains details for all users who have registered upto but not including 7th April 2008.

5. Select 'Save Target As...'

6. Save file

Once saved, the file doesn't need downloading again until the next quarters results are available

How it works

On a quarterly basis, the following data is supplied by the laboratories to QA: (1) Date Sample Taken; (2)Where Sample was taken - this is the location code used by the laboratory to identify the practice/clinic/department; (3) Who took the Sample - this is the Sample Taker code used by the laboratory to identify the Sample Taker; (4) Result of the Sample - one of the 8 categories- Inadequate, Negative, Borderline, etc. Please note, the laboratory does not supply the reason for an Inadequate result, and it does not supply any patient data. The data is collated and input onto the Sample Taker Database. It holds a maximum of 12 months data at any one time, which is reflected in the filename, for example 'smeartaker_database_010107_311207.xls' holds data covering the period 1st January 2007 to 31st December 2007.

NOTE - Sample Takers and Practice Managers download different files

Sample Takers - application shows 12 month data for individual for all locations

Practice Manager - application shows 12 month data for 1 practice

Using the application

Technical requirement - PC running Excel, with macros enabled. User will be prompted for file password - THIS IS NOT YOUR INDIVIDUAL PASSWORD - please refer to confirmation letter for details. User will then be prompted for their individual User Name and Password.

The Sample Taker Registration form is divided into 4 sections:-

1. Name and correspondence address - this is where QA will write to you.

2. Practice/Clinic/Department details - the form is divided into identical sections, to enable multiple work locations to be recorded. If the Sample Taker only takes Samples at one location then only 1 section needs to be completed.

3. Training History - used by QA to record training history.. Please note it is the PCT responsibility to prompt Sample Takers when training is due and to provide it.

4. User Name and Password - If Sample Taker wants to choose their own User Name and Password, complete this section. Both need to be 6 uppercase characters, anything longer than 6 will be truncated/amended. If this section is left blank, QA will supply these.

 

Practice Manager Registration form

The Practice Manager Registration form is divided into 3 sections:-

1. Name and correspondence address - this is where QA will write to you.

2. Practice/Clinic/Department details.

3. User Name and Password - If Practice Manager wants to choose their own User Name and Password, complete this section. Both need to be 6 uppercase characters, anything longer than 6 will be truncated/amended. If this section is left blank, QA will supply these.

Once these forms are received by QA, the details will be added to the database, and a confirmatory letter will be sent acknowledging receipt and detailing how and when the registered user can download the application.
